• Didn’t I tell that you should add me to the meeting?
• Didn’t you understand that you should add me to the meeting?
• Didn’t you not know how to add me to a meeting?
• Didn’t you want to add me to the meeting?

Outcome:

We gained knowledge why employee didn’t do what he was supposed to and are able to take the proper action to avoid this situation in the future.
